Worked for me but had to tell i tunes to import the folder first before sync.
Ringtone was added up the top in the “custom” section.

I have the exact same problem as van. Does anybody know why even though the iPhone in iTunes says that the new ringtone has been added properly, it still won’t show up in the ringtones tab of the sound options menu on the phone itself?

I had to buy one first to create ringtone folder in iTunes.. then this worked.. probably could have just created the folder (if I had known).
